How to start a physiotherapy clinic in India

How to start a physiotherapy clinic in India: practice models, registration, equipment and setup ballparks, a launch plan and how to build a referral base.

Day-1 tech rule: Exercise recovery plans, recurring appointment packages and posture tracking.

The short answer

A physiotherapy clinic can start small: a qualified physiotherapist, 400–600 sq ft, two or three treatment cubicles, an exercise area and core electrotherapy equipment. Register the business and premises, check your state's current rules for physiotherapist registration, and set up Shop & Establishment and municipal licences.

Patients rarely come for one session. They come for a plan of six to twelve, and most of your income depends on whether they finish it. Clear packages, easy rescheduling and home exercises they actually do will matter more than the newest machine.

Is a physiotherapy clinic right for you?

Desk jobs, two-wheeler commutes, weekend cricket, knee replacements and an ageing population all feed demand for physiotherapy in Indian cities. Many patients still see it as optional, though, so they drop out when the pain eases — which is often before the problem is fixed.

The founders who do well are physiotherapists with a clear specialty — sports, orthopaedic rehab, neuro, women's health or paediatrics — and a few doctors who trust them enough to refer.

  • Drop-offs. Patients stop after three or four sessions unless they understand the plan.
  • Referral dependence. Orthopaedic surgeons and GPs send many patients; that takes time and credibility to build.
  • Capacity. One therapist can only see so many patients a day, so pricing and scheduling decide income.
  • Home visits. Popular with seniors, but travel time eats into the day.

Do the capacity maths before you sign a lease. Count the treatment hours one therapist can realistically deliver in a week after documentation, travel and breaks, multiply by your session price, and compare that with rent and salaries. If the numbers only work at full occupancy, either the rent is too high or you need a second therapist and a plan to fill their diary. Many clinics close the gap with group classes — back care, post-natal strength, senior balance — which use the same space and expertise for several patients at once.

Professional indemnity insurance is not a licence but is worth having. So is a written consent form for treatments that carry any risk.

Registration rules for physiotherapists in India are evolving as national allied-health regulation is implemented. Confirm the current requirements with your state council or health department, a CA or a lawyer. This is not legal advice.

What it costs to start

Step by step: from idea to first customer

  1. Choose your specialty and the patients you want most — athletes, office workers, post-surgery, seniors.
  2. Map referral sources within 3 km: orthopaedic clinics, hospitals, gyms, sports academies and senior communities.
  3. Pick a site with ground-floor or lift access, space for gait training and somewhere to park.
  4. Register the business and check professional and clinical registration rules for your state.
  5. Buy core equipment first. Add specialised machines only when your patient mix proves the need.
  6. Design packages. Assessment, then 6-, 10- or 12-session plans with a clear price and validity.
  7. Write a cancellation policy that is firm but kind, and a reschedule process that takes seconds.
  8. Set up booking and reminders so patients never have to call to confirm.
  9. Meet referring doctors in person. Bring a one-page note on what you treat and how you report progress.
  10. Launch with free posture or ergonomics talks at nearby offices and gyms.

Running it day to day

  • Scheduling. Book in 30- or 45-minute blocks and never double-book a therapist. Leave buffers for documentation.
  • Outcome tracking. Record pain scores, range of motion and function at each visit. It keeps patients motivated and gives referring doctors a reason to trust you.
  • Home programmes. Short videos and written instructions for exercises between sessions. Patients who do them recover faster and finish their plans.
  • Records and privacy. Keep notes secure and share them only with consent. See our DPDP Act checklist.
  • No-shows. Reminders a day before and two hours before; a waiting list to fill gaps.
  • Cash flow. Collect package fees upfront by UPI, issue receipts, and track sessions used against sessions paid.

Getting your first patients

  • Doctor referrals. Send referring doctors a short progress note after the assessment and at discharge. That one habit builds a referral pipeline.
  • Google Business Profile. Services, fees, timings and photos of the space. Follow our local SEO guide.
  • Gyms and sports academies. Offer injury screening days and a referral arrangement.
  • Corporate ergonomics. Desk-posture workshops for nearby offices often turn into patients.
  • Ethical reviews. Ask patients at discharge. No paid reviews and no promises of cures in your ads.

If you also run classes, look at how to start a gym — the operating playbooks overlap more than you'd think.

The tech you need

A physio clinic needs less software than most clinics, but it needs the right pieces. Start with a booking page patients can use without an account, reminders on WhatsApp or SMS, and a simple record of sessions per package.

Our concept build Moviva is a single-practitioner physiotherapy site built around exactly that: an instant slot picker that prevents double bookings, no-login links to reschedule or cancel, a patient page with posture notes and personalised exercise videos, and a clinician desk for availability and blackout dates. It is the kind of product our doctor appointment app page describes, scaled to one clinic.

For more on booking systems, read our clinic appointment software guide. If you only need a site with a booking button, a business website may be enough. Brief us for a fixed quote within 48 hours.

We designed and built this concept end to end, every side of it, so you can open it and try it before you plan your own. Our doctor appointment app page covers the features, timeline and what moves the cost.

    Questions people ask

Can a physiotherapist open a clinic without a doctor?

Generally yes. A qualified physiotherapist can run an independent clinic, though patients are often referred by doctors and some treatments need a medical diagnosis first. Registration rules for physiotherapists are changing as national allied-health regulation is implemented, so check your state's current requirements before opening.

How much does it cost to open a physiotherapy clinic?

A small clinic with one therapist often needs somewhere around ₹8–20 lakh including deposit, interiors, core equipment and a few months of running costs. Specialty rehab centres with advanced equipment need much more. These are ballparks; get local quotes for rent and equipment.

How should I price physiotherapy sessions?

Look at what nearby clinics charge, then price a separate assessment plus session packages with a validity period. Packages help patients commit to a full plan and give you predictable income. Keep home visits priced separately to cover travel time.

Do I need a separate licence for home-visit physiotherapy?

Usually not a separate licence, but your professional registration, insurance and business registration must cover it, and you should keep the same records and consent as in the clinic. Check whether your state's clinical establishment rules say anything specific about home services.

How do I get referrals from orthopaedic doctors?

Meet them in person, explain what you treat, and send short progress notes after the assessment and at discharge. Doctors refer to physiotherapists who report back and whose patients improve. Never offer payments for referrals; it is unethical and damages trust.
